#LinkWorkerDay24 Awards Finalists

The National Association of Link Workers annual #LinkWorkerDay24 Awards now in its 5th year, celebrates the impactful contributions of Social Prescribing Link Workers and recognising their outstanding efforts and their partners, in transforming healthcare for all through social prescribing.

A huge thank you to everyone who nominated. Good luck to all the finalists that have made this year’s shortlist.

The winners will be revealed at the Awards ceremony which will be taking placing on 24th may part of our annual conference in Birmingham.

2024 Finalists:

Social Prescribing Link Worker of the Year

The Social Prescribing Link Worker of the Year award celebrates an exemplary professional who, in their role for a minimum of 1 year, embodies excellence in Social Prescribing Link Worker practice. This accolade recognises an individual who surpasses daily expectations, demonstrating transformative leadership, overcoming significant challenges, and showcasing tangible evidence of their impact in transforming healthcare for all. This award aims to recognise and honour Social Prescribing Link Workers who have demonstrated exceptional dedication and creativity in their role, contributing significantly to the improvement of healthcare accessibility and outcomes for all.

Social Prescribing Partnership of the Year

The Social Prescribing Partnership of the Year award recognises outstanding collaborative efforts in social prescribing services funded by entities such as Health and Social Care Partnerships, Integrated Care Systems, and Health Boards. This accolade celebrates systems partnerships that have not only achieved success but also embedded social prescribing into their system through innovative approaches, inclusivity, and strategic collaboration to benefit all parts of their system.

Only one nomination met the eligibility and judging criteria and will therefore be crowned winner at the awards ceremony.

  • The Life Rooms (Mersey Care NHS Foundation Trust) and Access Elemental Social Prescribing

Peer Support Worker of the Year

The Peer Support Worker of the Year award recognises an outstanding peer support group worker that has been either established by Social Prescribing Link Workers or receives referrals from them, showcasing a profound impact on the community. This accolade is designed to celebrate someone that, through their own initiative, address identified gaps in provisions, passionately tackle social needs, and have developed strong resilience to overcome challenges. To be eligible, the peer support group must have been in existence and operational for a minimum of 1 year. 

Only one nomination met the eligibility and judging criteria and will therefore be crowned winner at the awards ceremony.
